Chargemaster (CDM)

A comprehensive internal list of every service, procedure, supply, and item that a healthcare provider can bill for, along with the provider's standard price for each — before any contractual adjustments or patient discounts are applied.

The chargemaster — formally the charge description master, or CDM — is the master price list that a hospital or health system maintains for all billable items. It represents the provider's "retail" price before any contractual rate applies. For most commercially insured patients, the chargemaster price has little practical significance — the actual reimbursement is governed by the negotiated rate in the payor's contract, not the chargemaster. For self-pay patients or certain out-of-network situations, however, the chargemaster price can be the starting point for determining patient financial responsibility.

The relationship between chargemaster prices and negotiated rates

Chargemaster prices are typically set significantly above what any insurer actually pays — sometimes by 200 to 400 percent or more. This gap between billed charges and negotiated rates is a structural feature of hospital billing that reflects both historical pricing strategy and the multi-layered discount structure of commercial contracting.

For medical groups focused on payor contract strategy, the chargemaster is relevant primarily as context — it sets the ceiling from which payors negotiate discounts. The metric that actually matters for revenue optimization is the negotiated rate, not the chargemaster price.

Hospital Price Transparency and chargemaster disclosure

The Hospital Price Transparency rule (separate from the Transparency in Coverage rule applicable to insurers) requires hospitals to post their standard chargemaster prices publicly, along with their negotiated rates for a defined set of shoppable services. This disclosure has made hospital pricing more visible, though the data is primarily useful as a comparative reference rather than a direct guide to what insurers actually pay.
